This form is designed to help us place the right puppy with the right owner. Your assistance in filling out this form will help us to make the best choices for you and our pups. All information will be confidential and is for use in placing our pups only.

What type of dwelling?
Is anyone at home during the day to care for the pup?
If not can someone exercise/feed the pup mid-day?
Do you have a fenced yard?
If not fenced are you willing to put up fence or a suitable kennel?
Have you ever bred a litter of puppies?
Have you ever shown a dog in breed/obedience/agility?
Do you currently own any other pets?
Have you previously owned other dogs?
Have you ever returned an animal to the source from which you purchased it?
Have you ever given an animal to a pound, shelter or rescue?
Where will your dog primarily live?
Do all family members want a new Akita puppy?
Do any family members have allergies to dogs?
Do you prefer your puppy to be:
Are you willing to take this puppy to obedience classes?
Are you willing to purchase and use a dog crate?
If you have dominance or other problems with this dog, do you have the time and resources to work through them if these involve other family members, or other pets?
Are you committed to and prepared for taking care of this dog's needs for his entire life?
Are you familiar with the concept of raw feeding, sometimes known as BARF?
If you are not buying this puppy specifically as a show dog, are you willing to neuter him/her?
